Benefits
Medical Insurance
Employee receives medical insurance coverage for themselves, their spouse or domestic partner and their dependents, if they so choose. Employee is eligible the 1st of the month following 30 days of employment. Three levels of coverage are available through United Health Care: Choice Plus Basic, Choice Plus Core, and Choice Plus Premium.
Dental Insurance
Employee receives dental insurance coverage also if they have selected medical insurance coverage for themselves, their spouse or domestic partner and their dependents, if they so choose. Employee is eligible the 1st of the month following 30 days of employment.The UCFS Dental Plan is through Guardian Dental.
Flexible Spending
Employee may establish a Flexible Spending Account / Daycare Reimbursement on the 1st of the month following 30 days of employment. This benefit is a fiscal year commitment and funds must be used within the year or will be forfeited. It is also a voluntary pre-tax payroll deduction.
Retirement - 8.5%
Employee receives an 8.5% contribution from UCFS towards a 401K retirement plan. Employee is eligible to participate the 1st of month following employee's one year anniversary. Employee must work 20 hours or more per week, have completed 1,000 hours in that year. The vesting schedule for new employees is 5% of the employer contribution is fully vested upon eligibility. The additional 3.5% of the employee's wages are vested after 3 years of employment. All employee contributions are fully vested.
403B
Employee may enroll as of date of hire with Vanguard. Total tax deferred contributions may not exceed $10,500 per calendar year. This benefit is a pre-tax payroll deduction.
Long-Term Disability
Employee receives 60% of base annual salary while out of work during disability leave (90 day waiting period applies). Employee is eligible the 1st. of month following employee's one year anniversary. Employee must work 20 hours or more per week and have completed 1,000 hours in that year.
Life Insurance
Employee is insured for one time their base annual salary. Employee is eligible the 1st of month following employee's one year anniversary. Employee must work 20 hours or more per week and have completed 1,000 hours in that year.
EAP
Employee may access Employee Assistance Program services at anytime provided by the Department of Care Management at Backus Hospital in Norwich.
Holidays
Holidays are recognized as paid holidays for benefit eligible employees, they include: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Friday after Thanksgiving Day & Christmas Day.
Vacation
Vacation will be accrued at a rate of 3 - 5 weeks per year and maybe used by an employee after the initial introductory period of six months. In addition, Sick Time will also be accrued.
Voluntary STD
Employee may purchase a personal Short-Term Disability policy through AFLAC. This is optional and at full cost to the employee. Provides one a percentage not exceeding 60% of annual income in the event of a short term leave due to accident or illness. This benefit is a voluntary payroll deduction.
Voluntary Life Insurance
Employee may purchase additional life insurance from Guardian Life Insurance Company for self and immediate family members through payroll deductions.
