Brief Position Statement
United Community & Family Services has an opening in Norwich, CT and Jewett City, CT, for a Physician. Duties include diagnosing and treating a variety of diseases and injuries and providing continuity of care through maintaining admitting at the area hospital and participating in on-call rotation. Candidate will also perform responsibilities for geriatric assessment, the Agency’s management program, and nursing home consultation as needed.
Qualifications
Must possess an M.D. degree or its equivalent, a License to practice in the State of Connecticut, and at least 3 years as an intern in an accredited medical residency program. Must possess board certification or eligibility in a primary care specialty.

Pediatric LPN - Part-time
Brief Position Statement
The main purpose of this role is to assist with telephone & face to face patient communication and with physical exams & lab testing, as well as provide patient education and respond to patient needs. Join our team of Health Care professionals who provide client centered services in adult primary care, pediatrics and women’s health. Schedule is typically 1st shift Monday - Friday
Qualifications
- Graduate of an Accredited School of Nursing, with current CT State license and CPR certification
- Minimum of 1 year experience, preferably in Pediatrics &/or Outpatient Primary Care
- Demonstrated communications, diplomacy and problem solving skills, coupled with time management, organization and team building
- Exceptional client centered service provision and team & health care provider communications and collaboration skills
- Proficiency with Excel, PowerPoint, Access, Publisher, MS-Office & the Internet
- Bilingual – Spanish / English speaking candidates preferred

Outpatient Clinician
Brief Position Statement
Provide a broad range clinical outpatient services for individuals, families, couples or
groups of all ages regarding psychological, behavioral or emotional problems through the development and implementation of clinical assessment and clinical treatment plans.
Qualifications
- Masters degree required: MA/MSW/MFT
- License preferred: LCSW/LMFT/LPC, license eligible required
- 2 - 3 years of related clinical psychotherapy experience
- Ability to provide for strength based, client centered service delivery with a culturally diverse client base

Customer Service Representative – Colchester – Part-time
Brief Position Statement
The main purpose of this role is to provide courteous and professional client centered customer service to clients, visitors and callers of our Colchester Behavioral Health Services Outpatient site, while performing medical records and clerical duties, as well as supporting clinical providers and staff. This is a part-time position and the typical schedule would be Monday thru Wednesday, 4 – 8 p.m., Thursday, 4:30 – 8 p.m. and Friday 1 -8 p.m. This position may also require travel on-the-job 10% of the time.
Qualifications
- Ability to work the typical schedule stated above or similar
- High School Diploma/GED
- Minimum 2 - 3 years of related customer service, medical or behavioral health reception &/or medical records experience required; 3 - 5 years preferred
- Excellent demonstrated customer service and data entry skills with commitment to accuracy
- Demonstrated team player, someone who can support the staff, services and operations of a Behavioral Health outpatient environment with confidence & comfort in a positive and productive manner
- Demonstrated strength in organization, data entry, attention to detail and working with highly sensitive, protected confidential information and records, as well as multi-task
- Capable of required physical demand - When performing medical records tasks may frequently be required to lift and move records of up to 35 lbs. on a regular basis
- Valid CT State Drivers License in good standing and own self-insured reliable vehicle
