Eastern CT Medical Home Initiative for Children & Youth with Special Health Care Needs
Program Description
The Medical Home Initiative provides care coordination services to children and youth with special health care needs (CYSHCN) and their families/caregivers. Care coordinators work with children’s health care providers to link children and families to medical, educational and other community-based services. Help applying for respite and extended service funds is also available.
