Case Management

Provides support and coordination of services to clients with high-risk health conditions such as diabetes or asthma, or those struggling with basic needs such as stable housing, food, health insurance, employment and training or barriers accessing health care services. Case managers provide a confidential needs assessment and assistance connecting to health and community-based services to help clients improve their overall health.
